Software testing in the life sciences: more than bug fixing

In the traditional software context, quality assurance (QA) and software testing are often reduced to functional tests and bug fixing. Development delivers code, quality assurance  finds errors and ensures stable releases. However, this image proves insufficient in the life sciences sector. Here, quality assurance is not a downstream step, but a central component of a framework of regulatory requirements, scientific requirements and ethical responsibility.

Today, life science software is deeply embedded in safety- and patient-relevant processes — whether in electronic study databases, laboratory information systems or in the embedded control software of medical devices. In this GxP-regulated environment, an error not only has economic consequences, but also potential health consequences for patients. Regulation and compliance are therefore not downstream aspects, but shape how such systems are developed, operated and documented from the outset.

 

Regulatory Requirements Are an Integral Part of QA

A key difference between traditional quality assurance and QA in the life sciences sector lies in the regulatory context. Software in this area is not just a technical product — it is also a regulatory artifact. This means that it must be validated, fully traceable and auditable at all times.

Regulatory standards such as FDA 21 CFR Part 11, European Union GMP Annex 11 or international standards such as ISO 13485 go far beyond functional test requirements. They require systematic verification that a system reliably performs what it is intended for over its entire life cycle. This is typically achieved via formalized validation processes such as Computer System Validation (CSV) — a cornerstone of any quality management system (QMS) in the life sciences. CSV documents that a system works according to specification and delivers consistent results.

FDA 21 CFR Part 11 also specifies mandatory requirements for electronic records and signatures. The aim is to ensure that digital data is legally binding and trustworthy for regulatory purposes. Without this basis, the integrity of data submitted in regulatory submissions and approval procedures would simply not be provable.

 

Software Testing as a Systemic Process

Life sciences QA is not limited to detecting bugs. It encompasses a broad spectrum of activities that result directly from regulatory requirements:

  • Checking requirements for testability and regulatory relevance

  • Creation and maintenance of risk analyses

  • Definition and implementation of validated test plans

  • Ensuring traceability of requirements, tests and results

  • Documentation of all audit and inspection readiness activities

Validation approaches such as GAMP 5 (Good Automated Manufacturing Practice) have established themselves as the de facto standard in the pharmaceutical and MedTech industry. They combine risk assessment, requirements management, test execution and documentation into a consistent framework.

Traceability is particularly important here — end-to-end traceability from the requirements document to the test case to the audit report. Missing or incomplete traceability is one of the most common causes of regulatory findings during inspections. The principle always applies: what is not documented is formally considered non-existent. This is a fundamental difference to QA processes in less regulated industries.

 

Data Integrity and Patient Safety

Data integrity is a particularly sensitive issue in regulated QA. Clinical decisions, approval documents and safety-critical assessments are based on the reliability of underlying data — a principle captured by the ALCOA+ framework (Attributable, Legible, Contemporaneous, Original, Accurate). This is not a purely technical issue — it is an ethical and regulatory obligation at the same time.

Regulatory guidelines such as Good Clinical Practice (GCP) explicitly emphasize that clinical data must be recorded completely, consistently and comprehensibly — this is the only way to ensure that study results are valid and ethically usable. QA activities therefore include monitoring study data, ensuring consistent data flows and correlation checks between source and analysis systems.

In recent years, the issue of data integrity has become even more important. Digital and decentralized study designs generate significantly larger and more complex data volumes than traditional study formats. Regulatory authorities worldwide are therefore explicitly calling for risk-based approaches to data integrity management throughout the entire validated system lifecycle.

 

 

Methodological Depth: Risk-Based Quality Strategies

Another distinguishing feature from traditional software quality is the consistent use of risk-based testing strategies. Regulated standards do not require all functions to be treated equally — instead, the focus is on targeted prioritization:

  • Functions are evaluated according to their potential impact on patient safety, data integrity and compliance

  • Test strategies are defined based on this risk assessment, not on Intuition

  • Use of formal risk analyses such as FMEAs (Failure Mode and Effects Analysis)

Such approaches are an integral part of guidelines such as GAMP 5 and are increasingly favored by authorities because they concentrate QA resources where failures would have the greatest impact.

 

When Failures Reveal Systemic Weaknesses

In the life sciences industry, errors rarely occur as isolated technical problems. The strict regulatory framework means that defects often become visible as systemic weaknesses. A few typical examples from practice illustrate this:

  • Inconsistent study data can lead to clinical results not being accepted for approval decisions, resulting in significant delays or even rejections.

  • Incomplete documentation of a validated system can lead to regulatory findings during an inspection, even if the system functions technically flawlessly.

  • Usability deficiencies in laboratory software can lead to incorrect data entry — not because the software is defective, but because its operation invites misunderstandings.

These examples show that software quality assurance in the life sciences sector works simultaneously on a technical, regulatory and organizational level. It can only be effective if it is understood as an integral part of a complex, networked system — and not as an isolated control function at the end of a development process.
